Subarachnoid Haemorrhage

A haemorrhage into the subarachnoid space in the BRAIN. It is usually the result of rupture of an ANEURYSM on the CIRCLE OF WILLIS. Head injury or intense physical exercise occasionally cause subarachnoid haemorrhage; the diagnosis is con?rmed by CT scan or by identifying blood in the CEREBROSPINAL FLUID at LUMBAR PUNCTURE. Cerebral ANGIOGRAPHY will usually pinpoint the site of bleeding. Treatment is bed rest, life-support measures and procedures to reduce blood pressure; sometimes surgery is carried out but not usually until several weeks after the acute episode. About 30 per cent of patients recover fully, whilst some have residual disabilities such as EPILEPSY, mental deterioration or paralysis. Antepartum Haemorrhage

Bleeding from the vagina after the 28th week of pregnancy. Antepartum haemorrhage is most commonly due to a problem with the placenta, such as placenta praevia or placental abruption. Bleeding can also be caused by cervical erosion or other disorders of the cervix or vagina.

Admission to hospital is necessary for investigation and treatment.

Ultrasound scanning is used to diagnose problems with the placenta.

If the bleeding is severe, the woman is given a blood transfusion, and the baby is delivered immediately by caesarean section.... antepartum haemorrhage

Cerebral Haemorrhage

Postpartum Haemorrhage

Excessive blood loss after childbirth. It is more common after a long labour or after a multiple birth. The haemorrhage is usually due to excessive bleeding from the site where the placenta was attached to the uterus.... postpartum haemorrhage

Retinal Artery Occlusion

Blockage of an artery supplying blood to the retina, most commonly due to thrombosis or embolism, The disorder can result in permanent blindness or loss of part of the field of vision, depending on the artery affected and whether or not the condition can be treated quickly enough.... retinal artery occlusion

Retinal Detachment

Separation of the retina from the outer layers at the back of the eye. Detachment may follow an eye injury but usually occurs spontaneously. It is usually preceded by a retinal tear, and is more common in highly myopic (shortsighted) people and in people who have had cataract surgery.

The detachment is painless. The first symptom is either bright flashes of light at the edge of the field of vision, accompanied by floaters, or a black “drape” obscuring vision.

Urgent treatment is required and usually involves surgical repair of the underlying tear. If the macula (site of central vision) has not been detached, the results can be excellent.... retinal detachment

Retinal Vein Occlusion

Blockage of a vein carrying blood away from the retina.

It usually results from thrombosis in the affected vein, and is more common in people who have glaucoma.

Splinter Haemorrhage

Bleeding under the fingernails visible as tiny splinterlike marks.

Eyes – Foreign Body

From coal dust, insects, pollen, etc.

Symptoms: blinking, watering, acute discomfort. Sensation of grit in the eye does not always imply foreign body, but symptoms of conjunctivitis or keratitis. Automatic blinking is sometimes enough to clear offending object.

Treatment. External. Evert lid and remove. Swab out with dilute Witch Hazel on cotton wool. Inject one drop Castor oil, (also good for scratched cornea), Aloe Vera gel or juice. Fenugreek seed puree. Juice of Houseleek and dairy cream.

Difficult case. Removal of particles of iron or dust, apply mucilage of Slippery Elm powder to eye – patient lying on his back, a second person injecting it into corner of eye, the patient moving eye in opposite direction. Safe and healing. Clean eye and bathe with warm milk.

Eyes – Infection

Whatever the infection, dendritic ulcer, corneal ulcer, herpes simplex or stye, treatment should be internal as well as external.

A study carried out at Moorfields Eye Hospital, London, has shown that those who use extended-wear soft contact lenses are more likely to develop microbial keratitis than users of other lenses.

Treatment. Internal: Powders, Tinctures or Liquid Extracts. Combine Echinacea 2; Blue Flag 1; Goldenseal 1. Doses: Powders: 500mg (one-third teaspoon or two 00 capsules). Liquid Extracts 30-60 drops.

Tinctures: 1-2 teaspoons. In water, or honey.

Comfrey. To promote epithelial regeneration. Potential benefit far outweighs possible risk.

Evening Primrose capsules.

Topical. Alternatives. (1) Goldenseal Eye Lotion: 1 part Goldenseal root macerated in 40 parts distilled extract of Witch Hazel 2-3 days. Strain. 5-10 drops in eyebath half filled with warm water; douche. Wipe eyelids. (2) Aloe Vera juice or gel. (3) Moisten Chamomile teabag with warm water and fix over eye for styes, etc. (4) Bathe with Periwinkle minor tea: 2 teaspoons to cup boiling water allowed to cool and strain. (5) Elderflower water. The above to relieve pain, redness and gritty sensation. (6) Evening Primrose lotion. (7) Raw carrot compress to ripen stye. Nasturtium seed compress.

Eyes  - Macular Degeneration

Zinc and selenium, supported by doses of Vitamin E and amino acid taurine produced dramatic results in some cases; effect said to be due to antioxidant activity mopping up free radicals associated with degenerative diseases (Journal of Nutritional Medicine)

A preliminary therapeutic trial in patients with ageing macular degeneration or diabetic retinopathy showed that supplementation with Beta-carotene, Vitamin C, Vitamin E and Selenium halted the progression of degenerative changes and in some cases even brought some improvement. Eyes  - Night Blindness

Inability to see at night or in imperfect light due to a deficiency of visual purple (rhodopsin) in the rods at the back of the eye due to low level Vitamin A. Night myopia usually affects people during twilight. “One in five people are not fit to drive at night.” May occur in glaucoma and other eye disorders. Other causes: old age, free radical damage.

Alfalfa tea freely.

Eyes – Sight Deterioration

Presbyopia. General deterioration of the eye, usually from long- sightedness. A natural ageing process. Nutritional deficiency is a common cause, promoted by smoking, alcohol and denatured foods. Fennel eye compress. Steep teabag in cold water and apply.

Chickweed Lotion. Take a handful of Chickweed, wash well, crush with a rolling pin, infuse in two cups boiling water until cool. Use as a compress or in an eye bath two or more times daily.

Brain Haemorrhage

Bleeding within or around the brain that is caused either by injury or by spontaneous rupture of a blood vessel. There are 4 possible types of brain haemorrhage: subdural, extradural, subarachnoid, and intracerebral. Extradural and subdural haemorrhages are usually the result of a blow to the head (see head injury). The development of a split in the retina, usually caused by degeneration.

A tear is more common in people with severe myopia.

A retinal tear may also be caused by a severe eye injury.

Extradural Haemorrhage

Bleeding into the space between the inner surface of the skull and the external surface of the dura mater, the outer layer of the meninges. Extradural haemorrhage usually results from a blow to the side of the head that fractures the skull and ruptures an artery running over the surface of the dura mater. A haematoma (collection of clotted blood) forms and enlarges, causing an increase in pressure inside the skull and resulting in symptoms several hours or even days after the injury. Symptoms may include headache, drowsiness, vomiting, paralysis affecting one side of the body, and seizures. Untreated, extradural haemorrhage may be life-threatening.

CT scanning or MRI confirms the diagnosis.

Surgical treatment consists of craniotomy, draining the blood clot, and clipping the ruptured blood vessel.... extradural haemorrhage

Intracerebral Haemorrhage

Bleeding into the brain from a ruptured blood vessel. It is 1 of the 3 principal mechanisms by which a stroke can occur. It mainly affects middle-aged or elderly people and is usually due to atherosclerosis. Untreated hypertension increases the risk of intracerebral haemorrhage.

The ruptured artery is usually in the cerebrum. The escaped blood seeps out, damaging brain tissue. The symptoms are sudden headache, weakness, and confusion, and often loss of consciousness. Speech loss, facial paralysis, or onesided weakness may develop, depending on the area affected. Surgery is usually impossible; treatment is aimed at lifesupport and the reduction of blood pressure. Large haemorrhages are usually fatal. For the survivor of an intracerebral haemorrhage, rehabilitation and outlook are as for any type of stroke.

Subdural Haemorrhage

Bleeding into the space between the outer and middle layers of the meninges, usually following head injury. The trapped blood slowly forms a large clot within the skull that presses on brain tissue. The symptoms, which tend to fluctuate, may include headache, confusion, drowsiness, and one-sided weakness or paralysis. The interval between the injury and the start of symptoms varies from days to months. Diagnosis is by CT scanning or MRI. In many cases, surgical treatment is needed. This involves drilling burr holes in the skull (see craniotomy), so that the blood can be drained out and damaged blood vessels repaired. If treatment is carried out at an early enough stage, the person usually makes a full recovery. A subdural haemorrhage that is small and produces few symptoms may not require any treatment. Periventricular Haemorrhage

(PVH) a significant cause of morbidity and mortality in infants who are born prematurely in which bleeding occurs from fragile blood vessels around the *ventricles in the brain. Bleeding extending into the lateral ventricles is termed intraventricular haemorrhage (IVH) and in severe cases can extend into the brain tissue (cerebral parenchyma).
